| Multi Purpose Health Supervisor, Class-3 | Successfully passed a minimum one-year Multi-Purpose Health Worker (MPHW) Basic Course or Certificate/Diploma Course from a Government recognized institution. |

Selected candidates will receive a fixed salary of ₹26,000 per month for the first five years. Thereafter, they will be eligible for regular appointment in the Pay Level-4 scale of ₹25,500–₹81,100.
